Case Manager needed for New Yorks largest harm reduction/syringe exchange program. Its mission is HIV prevention and comprehensive services for those at risk for,or infected with HIV. They serve approximately 20,000 people each year in the Bronx and East Harlem using a unique street side service delivery model. There are offices,in the Bronx and in East Harlem. Case manager would assess the psychosocial health status and needs of participants in the ANIMO Program. Provide concrete case mngmnt services to all enrolled in the ANIMO program,including eligibility for assistance,food/shelter needs,medical,transportation,mental health/substance use assistance etc. Serves as advocate,translator and educator for participants in proper use of community services. Knowledge of the Bronx and East Harlem IDU communities and service providers is required. Must have experience in the areas of HIV prevention,STDs,reproductive health and substance use. Must have experience working in a harm reduction environment in which many staff come from the communities the agency serves. Must know and support harm reduction philosophy. Must be willing to work outdoors at our street sites,do outreach,and work on holidays with time off at another time. Tuesday through Saturday schedule,hours vary. Must be able to work as part of a team within a larger organization of multiple programs. Should be an organized,selfmotivated person,committed to working in a multicultural,multiracial diverse environment. Bilingual Spanish REQUIRED.
